  • Updated questionnaire from teachers - Teacher Questionnaire Form
  • Current GP referral
  • Child's Medicare card
  • Child's most recent school reports
  • Recent assessment reports

Paediatric review and testing will take approximately 2-2.5 hours.
*Please ensure your child does not use hair gel or spray on day of appointment as this can interfere with brain mapping equipment.

Psychology Intervention appointments

Psychology intervention appointments are 50 minutes in duration and provide individualised strategies to support a child and family's needs. Initial appointment often requires a parent-only session to attain background information and explore parents' concerns and goals. It is ideal if both parents can attend.  Subsequent appointments will predominantly be with the child/adolescent and feedback will be provided to parents/carers at conclusion of session.

Please bring previous assessments and investigations by other specialist medical or allied health professionals.

Cancellation Policy

Whilst we understand that unplanned situations do arise, we encourage you to contact our team as soon as possible to advise of your situation. It is crucial that at least 3 business days' notice are provided to postpone/cancel an appointment. This is to ensure children on our waitlist have the opportunity to be seen. Our Paediatricians, Psychiatrists and allied health team are committed to supporting your children’s needs. When a patient does not provide adequate notice, other children lose their opportunity to be seen.

  • For new patients, a non-refundable deposit will be taken at time of booking. Any rescheduling of a booking requires at least 3 business days' notice, otherwise a further non-refundable deposit will be charged at time of re-booking.
  • For review patients, any rescheduling or cancelling of a booking requires at least 3 business days’ notice, otherwise a further late notice fee will be charged at time of re-booking.

In some circumstances, where a pattern of last minute cancellations or non-attendance emerges, further appointments will only be secured with full prepayment of expected fees.

Payment Policy
